Is PCOS a Cause of Infertility?

Estimated Reading Time: 8 minutes

Polycystic Ovary Syndrome (PCOS) is a complex endocrine disorder affecting up to 20% of women during their reproductive years (Palomba et al., 2015). It is one of the leading causes of infertility in women, though not every case of PCOS results in infertility (Joham et al., 2014). Understanding the relationship between PCOS and infertility is key to addressing this condition holistically and effectively. Ayurveda offers insights into PCOS through the lens of doshic imbalances and aims to restore reproductive health by addressing the root causes rather than merely managing symptoms (Lad, 2002).

How Does PCOS Affect Fertility?

PCOS interferes with normal ovarian function, which can hinder conception. The ovaries produce higher-than-normal levels of androgens (male hormones), disrupting the menstrual cycle and causing multiple follicles to remain immature, leading to anovulation (lack of ovulation) (Azziz et al., 2016). Without ovulation, conception becomes challenging.

Key Ways PCOS Contributes to Infertility:

1. Anovulation and Irregular Menstrual Cycles

  • PCOS disrupts the production of follicle-stimulating hormone (FSH) and luteinizing hormone (LH), which are essential for follicle development and ovulation (Teede et al., 2018).
  • Women with PCOS often experience irregular periods, making it difficult to predict fertile windows and increasing the challenges of conception.

2. Poor Egg Quality

  • Even when ovulation occurs, the quality of eggs may be compromised (Palomba et al., 2015).
  • Hormonal imbalances can interfere with maturation of eggs, leading to lower chances of successful fertilization.

3. Endometrial Issues

  • The uterine lining (endometrium) may not develop properly in women with PCOS, reducing the chance of implantation of a fertilized egg (Joham et al., 2014).
  • Irregular cycles can leave the endometrium inadequately prepared for pregnancy, affecting embryo development and implantation.

4. Insulin Resistance and Inflammation

  • Insulin resistance is common in women with PCOS, increasing the risk of obesity and metabolic disorders, which further reduce fertility (Azziz et al., 2016).
  • Chronic low-grade inflammation caused by PCOS affects the reproductive system, interfering with ovulation and implantation (Teede et al., 2018).

Not All Women with PCOS Are Infertile

While PCOS can affect fertility, many women with PCOS are still able to conceive with lifestyle modifications, appropriate medical treatments, and supportive care (Teede et al., 2018). Some women with milder cases of PCOS may experience occasional ovulation, allowing for natural conception. Others may benefit from ovulation induction or fertility treatments such as In Vitro Fertilization (IVF) (Palomba et al., 2015).

Ayurvedic Perspective on PCOS and Infertility

In Ayurveda, PCOS is seen as a result of imbalance in Vata, Pitta, and Kapha doshas. Specifically, Kapha dosha leads to the accumulation of excess tissue and hormones, contributing to cyst formation, while Vata imbalance affects reproductive channels (Sharma & Dwivedi, 2017). Ayurvedic treatment for PCOS focuses on restoring hormonal balance, enhancing ovulation, and nurturing reproductive health.

Key Ayurvedic Approaches:

  1. Balancing Doshas:
    • Kapha-pacifying diet and lifestyle changes aim to reduce hormonal disruptions and improve metabolism.
    • Vata-balancing routines help restore healthy menstrual cycles and reproductive function (Lad, 2002).
  2. Detoxification:
    • Gentle detoxification therapies eliminate Ama (toxins) from the body, improving hormonal balance and insulin sensitivity (Bhattacharya & Ghosh, 2021).
  3. Fertility Support:
    • Ayurveda emphasizes building Ojas (vital energy) through nourishing foods, meditation, and stress management to support reproductive health (Frawley, 2000).
  4. Emotional and Mental Health:
    • Stress is known to impact fertility. Ayurvedic practices like yoga, pranayama, and mindfulness help women with PCOS manage stress, promoting hormonal harmony (Dwivedi & Tripathi, 2019).

Can Women with PCOS Get Pregnant?

Yes, pregnancy is possible for women with PCOS, but it may require personalized care and consistent effort. For many, lifestyle adjustments and weight management can restore ovulation and improve fertility (Joham et al., 2014). Integrating Ayurvedic treatments with conventional care provides a holistic path to parenthood, enhancing the chances of conception and sustaining a healthy pregnancy (Bhattacharya & Ghosh, 2021).

How to Know When You Have PCOS?

Polycystic Ovary Syndrome (PCOS) is one of the most common yet underdiagnosed hormonal disorders in women. Many women live with PCOS for years without realizing it, attributing symptoms like irregular periods, weight gain, or acne to stress or lifestyle changes. However, early diagnosis and treatment are crucial to prevent long-term complications like infertility, diabetes, and heart disease.

At EliteAyurveda, under the guidance of Dr. Adil Moulanchikkal, we focus on root-cause diagnosis and holistic Ayurvedic healing, ensuring complete hormonal balance and metabolic restoration.

📖 Recognizing PCOS: The Key Symptoms

PCOS affects hormones, metabolism, and reproductive health. It manifests in different ways, but here are the most common warning signs:

✔ Irregular Periods – Skipped, delayed, or very heavy periods
✔ Excess Facial & Body Hair (Hirsutism) – Unwanted hair on chin, upper lip, chest, or abdomen
✔ Unexplained Weight Gain – Sudden weight gain, especially around the belly
✔ Severe Acne & Oily Skin – Persistent breakouts, even in adulthood
✔ Hair Thinning & Scalp Baldness – Hair fall and widening of the hairline
✔ Fatigue & Low Energy – Feeling constantly tired or weak
✔ Dark Skin Patches (Acanthosis Nigricans) – Dark, velvety patches on the neck, underarms, or groin
✔ Mood Swings, Anxiety & Depression – Emotional instability, irritability, or stress
✔ Fertility Issues & Difficulty Conceiving – Irregular ovulation leading to infertility

🔬 How Is PCOS Diagnosed?

Since PCOS presents differently in every woman, there is no single test to diagnose it. A combination of symptoms, blood tests, and ultrasound helps confirm PCOS.

1⃣ Blood Tests for PCOS

✔ Hormone Tests:

  • 🔹 High Testosterone Levels – Causes acne, hair growth, and scalp thinning
  • 🔹 LH (Luteinizing Hormone) & FSH (Follicle-Stimulating Hormone) Ratio – Imbalance indicates ovulation issues
  • 🔹 High DHEA-S – Linked to excess androgens (male hormones)

✔ Metabolic Tests:

  • 🔹 Fasting Insulin & Blood Sugar Levels – Checks for insulin resistance and diabetes risk
  • 🔹 Lipid Profile (Cholesterol Test) – Identifies heart disease risk

✔ Inflammation Marker:

  • 🔹 C-Reactive Protein (CRP) – High levels indicate chronic inflammation

2⃣ Ultrasound Scan (Pelvic USG)

✔ Findings in PCOS:

  • Multiple small cysts on ovaries (polycystic appearance)
  • Enlarged ovaries due to hormonal imbalance

⚠ Note: Not all women with PCOS have cysts, and some women without PCOS may have cysts. A proper diagnosis is based on a combination of symptoms, blood tests, and ultrasound results.

🌿 Ayurvedic Perspective on PCOS Diagnosis

In Ayurveda, PCOS is seen as a Kapha-Vata disorder caused by:
✅ Kapha accumulation → Weight gain, insulin resistance, slow metabolism
✅ Vata imbalance → Irregular cycles, infertility, anxiety
✅ Agni (digestive fire) weakness → Poor digestion, toxin buildup, hormonal disruptions

Ayurvedic diagnosis focuses on Nadi Pariksha (pulse diagnosis), symptom analysis, and Dosha evaluation to understand the root cause of hormonal imbalance.

🛑 When Should You See a Doctor?

If you notice two or more symptoms of PCOS, consult a doctor immediately. Early diagnosis can prevent complications like:
🚨 Infertility & Miscarriages
🚨 Type 2 Diabetes & Prediabetes
🚨 High Blood Pressure & Heart Disease
🚨 Sleep Apnea & Chronic Fatigue

At What Age Can You Develop PCOS? Understanding the Onset & Prevention

Polycystic Ovary Syndrome (PCOS) is one of the most common hormonal disorders affecting women. While PCOS is often associated with reproductive-age women, it can develop at any stage after puberty, and early detection is crucial for prevention and reversal.

At EliteAyurveda, under the guidance of Dr. Adil Moulanchikkal, we approach PCOS through Ayurvedic root-cause healing, helping women balance hormones naturally without lifelong medications.

📖 When Does PCOS Develop?

PCOS typically manifests during teenage years or early adulthood, but lifestyle factors, genetics, and metabolic imbalances can lead to its development at any stage of life.

Age GroupRisk of PCOS OnsetCommon Symptoms
Teenage Years (12-19 years)Early onset due to puberty-related hormonal shiftsIrregular periods, acne, weight gain, excessive hair growth
Reproductive Age (20-35 years)Most common diagnosis ageInfertility, insulin resistance, difficulty losing weight
After 35 YearsHormonal shifts, metabolic decline, stress-induced PCOSHair thinning, obesity, high cholesterol, prediabetes

(PCOS symptoms include excessive weight gain and menstrual disturbances.)
Charaka Samhita

🔍 Why Does PCOS Develop?

PCOS is a result of hormonal imbalances that disrupt ovulation and metabolism. Ayurveda identifies three primary causes:

1⃣ Kapha Aggravation & Insulin Resistance

✔ Excess Kapha Dosha leads to slow metabolism & weight gain
✔ Insulin resistance increases belly fat & hormonal imbalances

2⃣ Pitta Imbalance & High Androgen Levels

✔ Elevated testosterone leads to acne, facial hair, & hair loss
✔ Pitta dominance causes irregular cycles & mood swings

3⃣ Vata Imbalance & Poor Digestion

✔ Vata disturbances cause stress, irregular appetite, & gut issues
✔ Weak Agni (digestive fire) leads to toxin (Ama) accumulation

🛑 Early Warning Signs of PCOS

✔ Irregular or skipped periods
✔ Excessive facial & body hair growth (hirsutism)
✔ Acne & oily skin
✔ Unexplained weight gain (especially belly fat)
✔ Thinning scalp hair or hair loss
✔ Mood swings & anxiety
✔ Dark patches on the neck & underarms (insulin resistance sign)

If you experience these symptoms, early intervention is key to prevent complications like infertility, prediabetes, and metabolic disorders.

Let’s begin with the basics… What is polycystic ovary syndrome? (PCOS), And if untreated does it leads to cancer?

PCOS is a hormonal issue that influences women of reproductive age. It’s regularly portrayed by the abundance of Androgen (male hormones) levels and menstrual periods that are either infrequent or are longer than they usually are.

With PCOS, your ovaries may create follicles, which are little assortments of liquid, and they may neglect to routinely release eggs. High levels of male hormones produce symptoms like unpredictable ovulation or no ovulation at all. In straightforward words, women with PCOD or PCOS don’t release a mature egg from their ovaries consistently bringing about infertility. PCOS affects one out of 10 ladies worldwide.

Women suffering from PCOS experience the ill effects of symptoms like heavy vaginal bleeding. This is on the grounds that, as they don’t ovulate and deliver the hormone-Progesterone like normal women do and have typical periods, the lining of their uteri turns out to be thickly prompting heavy bleeding and, in a few cases, Uterine Cancer.

Normal Symptoms of PCOS:

  • Skin inflammation is also known as Acne.
  • Weight gain.
  • Oily skin.
  • Heart problems.
  • Irregular periods.
  • Headaches and Migraines.
  • Hirsutism- Unwanted hair growth for women.
  • Infertility.
  • Mood swings and episodes of depression.

How Ayurveda Treatment Can Help?

In Ayurveda, PCOS is caused because of an imbalance of any of the two doshas – Pitta (Fire) and Kapha (Water). Aggravated Pitta causes defilement of the dhatus or tissues like blood and plasma. This causes a build-up of toxins or ama in the body.

In PCOS patients, these toxins get accumulated in the channels of the psyche, prompting an imbalance of hormones discharged by the pituitary organ. This results in an irregularity of female hormones and causes the development of ovarian growth which is only an assortment of fluids inside the ovary, anovulatory periods or periods with no ovulation, and so on.

Remedies for PCOS/ PCOD

  • Correcting hormonal imbalances.
  • Reducing Obesity.
  • Reducing elevated cholesterol levels.
  • Treatment for insulin opposition.

Ayurvedic Herbs like Ashoka (saraca asoca), Dashamoola are amongst a group of herbs that are used to prepare Sukumara Kashaya like Ashwagandha, Eranda, Shatavari, etc for correcting hormonal imbalance.

Additional Risks if Ignored:

Many medications only mask the symptoms of PCOS without treating the underlying hormonal balance. These, if left ignored cause major health issues. Some of them are:

  • Type 2 diabetes

Insulin resistance affects a large percentage of women suffering from PCOS. Even though weight gain worsens your insulin resistance, you can still have a problem with your insulin if you’re lean.

Because insulin sensitivity decreases in women suffering from PCOS, Type 2 diabetes is a big concern, especially since the onset of diabetes will worsen your hormonal imbalance even further and make your PCOS management journey more difficult.

  • Heart disease

The biggest cause of mortality in the world is heart disease, and PCOS can increase your chances of suffering from it.

As PCOS comes with insulin resistance and elevated blood sugar levels, a domino effect occurs. Your bad cholesterol goes up and so does inflammation, providing the perfect environment for fatty plaque to appear on the walls of your blood vessels and increase your risk of ruptures and blood clots.

  • Endometrial cancer

can PCOS cause cancer?

Each month during menstruation, you shed the lining of your uterus. But when you have too much estrogen and too little progesterone (very common in PCOS), this process doesn’t occur, leading to very light periods or no menstruation at all.

Women who experience missed or irregular periods are at a higher risk of developing PCOS endometrial cancer.

  • Infertility

Hormonal imbalances are a big cause of infertility. If you have too much or too little of one hormone, your body won’t be able to generate or maintain a pregnancy.

Fortunately, it’s possible to treat PCOS-caused infertility. Most women with PCOS are able to get pregnant with the help of the right medication and lifestyle changes.

  • Non-alcoholic fatty liver disease

If you’re suffering from PCOS, you may be at an increased risk of developing liver disease, especially if you’re overweight or obese.

However, obesity alone isn’t the only concern. Even if you’re at a healthy weight, if your androgen levels are too high, this may contribute to the accumulation of fat in your liver.
