Nephrotic syndrome is a kidney disorder in which a patients’ body tends to release too much protein in the urine. This condition is caused due to the damage in the clusters of small blood vessels which are present in the kidney that filters waste and excess water from the blood. The symptoms associated with nephrotic syndrome are- edema in eyes, ankles and feet; foamy urine, weight gain, fatigue and loss of appetite. The cause for this condition may be diabetic kidney disease, minimal change disease, focal segmental glomerulosclerosis, membranous nephropathy, systemic lupus erythematosus, amyloidosis, etc.
According to ayurveda, nephrotic syndrome is caused due to the blockages in minute channels of the body called as Srotas in kidneys. These channels are known as mutravaha srotas which carry urine and are responsible for the flow of liquid into and out of the kidney. When these srotas are blocked or when they are shrinking, the kidneys tend to swell. This is caused due to vitiation of doshas, specifically Pitta dosha.
The treatment for nephrotic syndrome in ayurveda focuses on curing the root cause of the condition. In this case, due to blockages in the channels, the panchakarma procedure is accompanied, to clear off the toxins accumulated in Srotas. Further, patients are administered with herbal formulations that help in pacifying the effects of accumulated toxins. These medications remove obstructions from the passage of blood, help in nourishment of tissues, and cleansing of the system. This line of treatment helps in reversal of the condition in a holistic approach.

We can equate the swelling in nephrotic syndrome to some form of "Shotha”, which in ayurveda means "swelling." When it comes to treating the illness, the major focus is diverted on removing the swelling.

Swelling is only one of the symptoms. This isn't the cause. This is why, when discussing nephrotic syndrome, I left out the symptoms. Because the treatment isn't simply about alleviating a few symptoms. The sole option for treatment, wherever and whenever it is required, is to reverse the disease. And there is no singular pathophysiology for a disorder like nephrotic syndrome.

Further, because the three doshas are central to both Ayurvedic physiology and disease, it's critical to comprehend how these factors play a role in every sickness.

  1. Nephrotic syndrome and Vata dosha: The kidney is the Vata dosha's seat. As a result, the local dosha is Vata, which makes Vata Dosha an essential factor to consider while treating nephrotic syndrome. The foamy urine is also a symptom of Vata illness. Vata is a kind of dosha that causes tissue deterioration. The problem arises as a result of tissue damage.
  2. Nephrotic Syndrome and Pitta Dosha: Pitta dosha is responsible for tissue inflammation and deterioration. When Pitta takes over the role of Vata, Vata aggravates Pitta, exacerbating the condition.
  3. Nephrotic Syndrome and Kapha Dosha: The Kapha Dosha has something to do with water retention. The wastes in the urine rise when Kapha is completely absent.

The main objective of ayurvedic medications is to remove the ama from the passages of the blood; help in rejuvenation of the scar cells and cleanse the entire system.

The therapies of Basti and Virechana are employed in this condition. They help in elimination of toxins from the entire body and restore the functions of the kidney.

Nephrotic syndrome

There are certain signs and symptoms that your kidney isn't performing correctly. And when all of these factors come together, you have nephrotic syndrome. The following are some of them:

  • Albuminuria is a condition in which the kidneys are unable to retain proteins, and these proteins are flushed out in large amounts in the urine.
  • Hypoalbuminemia—the body suffers from a shortage of albumin in the blood due to the same loss of albumin in the urine.
  • Hyperlipidemia is a condition in which fat and cholesterol levels in the body exceed their normal range.
  • Swelling in the lower body regions such as the legs and feet, particularly around the ankle joint, is linked to the three disorders listed above. This swelling might also arise on the hands and cheeks in some conditions.

Causes of nephrotic syndrome

Primary causes of nephrotic syndrome: Damage to the glomeruli is the most prevalent cause of primary nephrotic syndrome. As there is formation of scars in the glomeruli, the kidney gets injured. Further, albumin leakage occurs when these scar tissues are inflamed. Antibodies may become lodged in the regions, causing damage and nephrotic syndrome. Different medical names are used to describe each of these disorders, which can be noted as below:

  • Minimal change disease- The developmental problem
  • Focal segmental glomerulosclerosis- scar tissues in the glomeruli
  • Membranous glomerulonephritis- inflammation of the membrane of kidney
  • Membranoproliferative glomerulonephritis- Deposition of antibodies in the kidney.

Secondary causes of nephrotic syndrome: Conditions that are noted as secondary causes of nephrotic syndrome can be noted as below:

  • Diabetes
  • Systemic lupus erythematosus disease
  • Sjogren’s syndrome
  • Cancers
  • Multiple myeloma
  • Amyloidosis
  • HIV
  • Vasculitis
  • Hepatitis B
