Scleroderma is an aggregate of autoimmune diseases that results in changes within the skin, blood vessels, muscles, and internal organs. The symptoms may include thickening of skin area, stiffness, fatigue, and poor blood flow to fingers or toes. Scleroderma is caused due to genetic and environmental factors. By genetics, there are mutations in HLA genes, and environmental factors like exposure to silica, aromatic & chlorinated solvents, welding fumes, white spirits, etc. contribute to the prognosis of the condition.

“ In Ayurveda, Scleroderma is termed as Uttana Vatarakta. It afflicts the superficial tissues which comprise skin & muscles. It appears to be Kushta (skin disease) with skin lesions and muscle pain. It is caused due to aggravation of Vata dosha due to intake of improper foodstuffs or long-distance rides on animals. Further, the Rakta gets vitiated due to intake of lavana, amla, katu, kshara, etc., and blocks the passage of Vayu. The Rakta dhatu vitiated by Vayu burns the whole blood in the body, which then gravitates towards the foot, thus leading to Uttana Vatarakta.

Ayurvedic treatment of Scleroderma involves treatments that involve the elimination of the root cause of the condition. This involves therapies of Lepa, Abhyanga, Parisheka, Avagahana, and Upanaha. The treatment also involves Shodhana therapy & Shamana therapy. Along with these external therapies, internal medication in the form of a combination of herbs is also given. This includes Vasadi Kwatha, Navakarshika Kwatha, Guduchi Kwatha, Kasmaryadi Kwatha, Patoladi Kwatha, Kokilakshyadi Kwatha, Laghu Manjishtadi Kwatha, etc. This line of treatment helps in the reversal of the Scleroderma condition in a holistic way.

Types of Scleroderma

There are majorly two types of Scleroderma, which have their own subtypes as mentioned below:

  1. Localized Scleroderma:

This type affects only the skin and not the major organs

  • Circumscribed Morphea

This comes with single oval patches or a few patches of morphea on the skin. They have a typical red border and thickened pale-yellow center. These lesions may enlarge when active and later flatten and vanish with proper treatment.

  • Generalized Morphea

This comes with many patches of morphea on the trunk, arms, head, or neck. They are seen as thick, tight areas that might mix together. Pansclerotic morphea is a severe kind of widespread morphea that affects the majority of the body.

  • Linear Scleroderma

It is most common in children of age 10 and younger. The tight and thick bands appear on the trunk, buttocks, or on the face. They appear as a single band with mixed patches of morphea. It can affect the bones and muscles.

  1. Systemic Scleroderma:

This type affects the skin, and tissue under it including blood vessels, and organs like the gastrointestinal tract (GTI), heart, lungs, or kidneys. 

  • Limited Scleroderma

It is the most common type of Scleroderma that leads to skin hardening and tightening in fingers, hands, forearms, or the face. The problems with the gastrointestinal tract, Raynauds, and musculoskeletal pain are usually seen in people with limited scleroderma. 

  • Diffuse Scleroderma

It causes excess collagen production which leads to thickening of the skin on fingers, hands, arms, anterior trunk, legs, and face. There is a risk of serious organ damage, including damage to the gastrointestinal tract, kidneys, lungs, and heart. Skin tightness is frequently coupled with dryness and irritation. Musculoskeletal discomfort is quite prevalent.

  • Sine Scleroderma

It causes systemic organ disease including the Raynaud's phenomenon. This does not include skin thickening and includes other symptoms of Limited and Diffuse Scleroderma.

Symptoms of Scleroderma

  1. Cardiovascular symptoms

This involves Raynaud’s phenomenon, healed pitting ulcers on fingertips, telangiectasis, palpitations, hypertension, and congestive heart failure.

  1. Digestive

This involves gastroesophageal reflux disease, indigestion, bloating, loss of appetite, constipation, sicca syndrome, loosening of teeth, etc.

  1. Pulmonary

This involves shortness of breath, chest pain, persistent cough, interstitial lung disease, etc.

  1. Musculoskeletal

This involves muscle ache, joint pain, carpal tunnel syndrome, and weakening of muscles.

  1. Genitourinary

This involves dyspareunia, erectile dysfunction, kidney failure, etc.

  1. Other

This involves stroke, fatigue, calcinosis, hand paresthesias, headache, weight loss, etc.

Ayurveda on Scleroderma

In Ayurveda, Scleroderma is termed as Uttana Vatarakta. It afflicts the superficial tissues which comprise skin & muscles. It appears to be Kushta (skin disease) with skin lesions and muscle pain.
