Frozen shoulder is also known as adhesive capsulitis which is the condition characterized by stiffness and pain in the joint of the shoulder. The symptoms usually begin from mild pain in the shoulder, which gradually increases and worsen over the time. It takes time for pain to get calm, i.e. around one to three years. In this condition, the shoulder capsule thickens and becomes tight. The thick bands of tissues keep on developing and thus, there is reduction in synovial fluid in the joint. This cause’s severe pain due to increase in friction and a person is unable to move shoulder.
‘Apabahuka’ is the term for frozen shoulder in Ayurveda. It usually affects amsa sandhi and is produced by Vata dosha. It is considered as Vataja vikara which is caused due to dryness of sleshaka kapha from amsa sandhi. Apabahuka is manifested due to depletion of tissue elements as well as samsrushta dosha. This involves painful movements of hands and stiffness in the joints of the shoulder, where the Vata gets accumulated.

The Ayurveda suggests the treatment for frozen shoulders in the following steps: Oleation, swedana, basti and nasya karma. Along with this Ayurveda recommends a combination of healthy diet and lifestyle changes that will help in management of the condition and by avoiding the vitiation of doshas further. Types of frozen shoulder

  1. Primary adhesive capsulitis: The causative reasons for primary adhesive capsulitis include changes in immune system, hormonal imbalances, and diseases such as diabetes, cardiovascular disease and neurological disorders. This condition may affect both shoulders and may be resistant to most forms of treatments.
  2. Secondary adhesive capsulitis: This condition is manifested from a known cause like stiffness that follows a shoulder injury, surgery or a prolonged period of inactivity.

Stages of frozen shoulder

  1. Freezing: this is the painful stage wherein the pain increases in the shoulder and makes it difficult to move. This pain worsens at night and can last from 6 weeks to 9 months.
  2. Frozen: At this stage the pain may decrease but the shoulder remains stiff. It lasts from 4-6 months and the movement of the shoulder is entirely restricted.
  3. Thawing: In this stage the movements start slowly owing to relief from pain. Pain in the shoulder may often fade and recur occasionally. This takes 6 months to 2 years to recover completely.

Treatment

  1. Snehana (oleation) is the process of using ghee, herbal oil, animal fat, or bone marrow to remove the body's uneven dosha.
  2. Application of hot, wet fluids to cure pain is known as swedana (fomentation).
  3. Enema (Basti) is a bowel cleaning treatment.
  4. Karma nasya (application of medicated oil in the nostrils)
  5. For external application, Ayurveda advises Mahanarayana taila, Vishagrabha taila, Bala Taila, and Prasaranyadi taila.

In order to properly manage the illness, Ayurveda also prescribes a mix of a balanced diet (ahar) and lifestyle adjustments (vihar).
