The lumbar spine, also known as the lower back, is a complex network of interconnected bones, joints, nerves, ligaments, and muscles that all work together to give stability, strength, and flexibility. However, because of its complicated anatomy, the back is prone to damage and discomfort. The most common cause of acute back pain is damage to the muscles, ligaments, joints, or discs. The body also mobilizes an inflammatory healing response in reaction to damage. While inflammation may appear to be small, it can cause excruciating discomfort. Because the nerve supply to many of the discs, muscles, ligaments, and other spinal structures overlaps, it might be difficult for the brain to determine which part is causing the pain.

According to Ayurveda, the back pain is the cause of vitiation of Vata dosha. The reason for vitiation of this dosha is unhealthy eating habits and lifestyle habits. This Vata dosha travels throughout the body and rests in the area of blocked channels, which in this case is in the back. This causes pain and stiffness in the bones, joints and ligaments.
In Ayurveda, the mode of treatment of back pain is to balance the vitiated dosha and eliminate the toxins accumulated in the channels. The treatment may include detoxification by various panchakarma procedures, abhyanga massage, herbal pottali massage, purgation and mild herbal enema along with classic ayurveda herbal medications. These procedures help in relieving pain and reversing the symptoms of back pain.

The Disc-Related Causes of Back Pain

Types of Back pain

Flexion dominant back pain: This type of back pain is often seen after disc injury whose symptoms include spine pain, leg pain, experience tingpng and numbness, the pain worsens by sitting, lifting and bending; and loss of range of motion.

Extension dominant back pain: Extension Dominant pain is frequently defined in terms of the zygapophyseal joints, which are located at the rear of the spine (or Z-joints). The symptoms may include pain after standing for longer duration; stiffness in back, pain after doing heavy impact activities, spinal pain, tingling or numbness, and leg pain. 

Neurogenic claudication: Neurogenic Claudication is a kind of Extension Dominant back pain that affects people over the age of 60. It occurs when the patient's nerves are crushed when standing or walking, and it can severely impede a patient's ability to walk for extended distances. The symptoms may include numbness and pain while walking; and weakness of muscles.

Inflammatory back pain: Back pain that is inflammatory in nature is less prevalent than mechanical back pain, although it is frequently misunderstood. Excessive inflammation in the spinal joints is caused due to the immune system attacking the joints of the spine. The symptoms may include- persistent pain for more than three months, back pain and stiffness worse when at rest, and easing of back pain with physical activities.

Chronic pain disorders: this type of back pain is accompanied by pain that is not fixed in frequency, but is persistent for years. The symptoms may include intense pain, the pain migrates to other parts of the body, pain is worsened by anxiety and depression, and pain is not localized.

Ayurveda about back pain

Spinal discs symbolise the Majja dhatu, one of the layers of sustenance necessary for the smooth movement of the spine. These cushions ensure that the spinal nerves can exit the spine at all levels. However, these cushions produce compression on nerves and problems when they shrink (degenerative disc ), deflate (bulging disc), or rupture (disc protrusion).

In all such circumstances, pain is the primary symptom. And pain is disc-specific, as it depends on just where the issue is. When a disc bulge occurs in the cervical region, it causes symptoms in the upper back and shoulders. And when a lumbar disc herniates, it affects the legs and feet.

It is the Vata dosha that causes problems when the Majja Dhatu becomes imbalanced. This is a well-written example of a popular concept in Ayurvedic texts. When a person's Majja Dhatu is imbalanced, he will have chronic Vata Dosha symptoms. Thus, we must take measures to calm the dosha

Symptoms of a Spinal Disc / Back problem

  • Low backache: Backache can be acute or chronic in nature.

Acute backache is the most severe. This form of pain strikes without warning. The fundamental cause of such acute disorders is muscular spasm and spine rigidity. However, in some situations, the rapid rupture of the disc might induce such acute symptoms.

The pain in chronic backache is dull and diffuse, and it is frequently exacerbated by exertion, forward bending, standing and sitting in one posture for an extended period of time. Resting helps to alleviate pain. However, if this is ignored for an extended period of time, it might lead to major problems.

  • Sciatica is a type of pain that extends through the gluteal region, the back of the thigh, and the leg. The radiation pattern is determined by the compressed root. The discomfort may radiate to the front of the thigh in some cases.
  • Neurological symptoms: The patient may complain of paresthesias, which is numbness in the leg or foot and muscle weakness. Lower limb paralysis and bowel incontinence occur when substantial disc debris compresses the roots.
  • Posture: The patient's lumbar spine is inflexible and flattened. The trunk as a whole slips forward on the hips, and the trunk can tilt to one side.
  • Movements: The patient is unable to bend forward; any attempt to do so causes significant muscle spasm in the paraspinal muscles.
  • Tenderness: Tenderness is present in the lumbosacral (lower) area. In some circumstances, local discomfort in the midline or lateral to the spinous process may occur.

How do spine / back problems occur?

  • A spinal disc problem is not a one-time occurrence; rather, it is the result of a series of changes in the disc that eventually leads to rupture or degeneration. These modifications include the following:


    • Degenerative changes in the nucleus are a common occurrence in the case of spinal disc disorders. These modifications occur in the disc before the nuclear material is displaced. These changes include softening and fragmentation of the nucleus, as well as weakening and disintegration of the annulus's posterior portion.
    • Nucleus displacement: The nucleus is constantly under pressure. This pressure weakens the fibres' outer ring (annulus fibre). The nucleus pulposus begins to protrude from within these weak and damaged fibres. Essentially, this is the condition known as disc protrusion.
    • Fibrosis stage: The body has a tendency to heal any damage. This is the repair stage. The remaining nucleus develops fibrosis and calcification. However, this type of restoration is unhealthy and worsens the situation.
    • Changes in structures inhabiting the spinal canal: The damaged nerve root is typically the one that exits the spinal canal beneath the following vertebra.
    • Intervertebral joint changes: The height of the disc decreases as a result of the loss of part of the nucleus pulposus and subsequent fibrosis. This impairs joint articulation and contributes to degenerative arthritis.

Ayurveda Treatment for Back Pain

Ayurvedic Medications 

Ayurveda does not recommend the use of painkillers to treat degenerative disc-related pain. Here, the primary goal of ayurvedic treatment is to restore balance to the aggravated doshas. Because, according to Ayurveda, these doshas cause spinal disc issues. When we balance the doshas, things begin to return to normal.

Panchakarma Treatment 

Panchakarma treatment is an excellent alternative to surgery for disc or radiculopathy issues. It is the ultimate therapeutic experience and is designed to revitalise the body. The treatment consists of Virechana, Basti, Kati basti, Pottali, Swedana, and Dhanyamla dhara, amongst others. These therapies aid in muscle relaxation by increasing blood circulation.

The line of treatment for backpain involves daily abhyanga with the medicated oils, detoxification by panchakarma procedures, and intake of herbal medications that help in repairing the swollen joints. Shodhana (purification therapy) and Shamana (palliative treatment) play a significant role in management of sandhi Vata. Along with this the person is suggested to practice pranayama and maintain a healthy diet to avoid vitiation of Agni and to resist the production of ama in the body.
