Crohn's Fatigue — The Symptom Nobody Can See

Tired Is Not the Word

You slept nine hours and woke up as though you had not slept at all. Climbing one flight of stairs feels like something you need to plan. You cancel things. You sit in the car for ten minutes before you can go into the building. And you have started to hide it, because when you describe it, people hear "tired", and everyone is tired.

This is not tiredness. This is a physiological state, and it has a mechanism.

Fatigue Is Not Just Anaemia

Almost everyone with Crohn's is told their fatigue is anaemia. Anaemia is part of it — and it should be corrected properly — but it rarely explains the whole picture. Many people have their haemoglobin restored and remain exhausted. If your doctor has fixed your iron and you still cannot function, you are not imagining it and you are not depressed by default.

Here is what is actually stacked on top of each other.

Inflammatory signalling acting on the brain. The inflammatory messengers produced in an inflamed bowel do not stay in the bowel. They circulate, they reach the brain, and they produce what researchers call sickness behaviour — the flattening, the withdrawal, the leaden limbs, the inability to think. It is the same machinery that puts you in bed with flu. In Crohn's it does not switch off. This is the largest and least acknowledged component of Crohn's fatigue, and it is why fatigue can be severe while your haemoglobin is unremarkable.

Iron deficiency, in two forms. You lose iron through bleeding. You also absorb it poorly — the duodenum, where iron is taken up, may be inflamed, and inflammation itself raises a hormone that shuts down iron absorption and locks iron away in storage. This is why ferritin can look normal or even high while you are functionally iron deficient. Ask your gastroenterologist for transferrin saturation alongside CRP, not ferritin alone.

B12 and folate. B12 is absorbed only in the terminal ileum — the classic Crohn's site. Ileal disease or resection means deficiency, with fatigue, brain fog and, if left, nerve symptoms.

Broken sleep. Nocturnal diarrhoea and night pain fragment sleep for months or years. Fragmented sleep raises inflammatory signalling, which worsens fatigue, which worsens sleep. It is a loop.

Undernutrition and muscle loss. Malabsorption plus a catabolic state strips muscle. Less muscle means more effort for the same task.

A dysregulated stress axis. Chronic illness alters cortisol rhythm, so you can be wired at midnight and flattened at 10am.

Some medications also contribute. Do not stop anything — ask your doctor.

Why This Matters More Than It Looks

Fatigue is usually the first thing to move when inflammatory load genuinely falls.

In our Recovery Stages, Recovery Stage 1 (weeks 1–4) is the Internal Shift: energy, sleep and digestion change before bowel symptoms do. People are often puzzled by this — they came for the diarrhoea and the first thing that lifts is the fog.

But that ordering is exactly what you would predict from the mechanism. Systemic inflammatory signalling is what produces the fatigue, and it is the first thing Phase L addresses. Mucosal healing takes months; the load that is making you feel ill can start coming down in weeks. So an early lift in energy is not a placebo-shaped consolation prize. It is the first honest signal that the load is falling — the earliest evidence you get that the direction of travel is right, long before a scope or a calprotectin can confirm it.

We say that carefully. It is a signal, not proof. It does not replace your objective monitoring, and you should keep every calprotectin, CRP and scope your gastroenterologist orders.

What Root-Cause Correction Does

Through LIFES:

  • Phase L — Inflammatory Load Reduction (oral, 4–8 weeks). The load that is producing the sickness behaviour comes down. This is where fatigue typically first responds.
  • Phase I — Internal Correction (oral, 8–16 weeks). Barrier repair, microbial restoration and immune recalibration — which is also what eventually restores iron and B12 absorption, so that supplementation finally works instead of passing through.
  • Phase F — Functional Clearance (oral, 6–12 weeks), only after L and I are stable — never first.
  • Phase S — Remission Maintenance (tapered oral, 6–12 months).

The Plateau

Around weeks 3–6, many people find the early energy gain stalls. This is the partial improvement plateau. It is the most common point at which people give up, days before the next stage of change. It is not failure, and it is not a sign the approach is wrong.

Honest Limits and Red Flags

If you have active severe disease, uncorrected anaemia, or an untreated B12 deficiency, no approach — including ours — will lift the fatigue until those are addressed. Those are conventional jobs and they come first.

Go to hospital now if you have breathlessness at rest, chest pain, fainting, or you have become very pale — those can indicate severe anaemia. Get medical review promptly for new numbness or tingling in your feet.
