Gut Health and HS — The Driver Furthest Upstream

The question nobody asked you

In all the appointments about your skin, did anyone ask about your bowels?

Whether you bloat after eating. Whether you alternate between constipation and urgency. Whether there is mucus, or blood, or pain, or fatigue after meals. Whether it has been like this since long before the first lump appeared.

Most people with HS are never asked, and a striking number of them, when finally asked, have a great deal to say. That is not coincidence. For a large proportion of HS patients the gut is the driver furthest upstream, which means it is the one where correction changes the most.

The gut-skin axis, mechanically

Your intestinal lining is a single layer of cells, and the whole of your immune education happens across it. Behind that layer sits the largest concentration of immune tissue in your body. The microbial community in the gut is what teaches that immune tissue what to tolerate and what to attack.

When the microbial community is disrupted, dysbiosis, two things follow.

The lining loosens. The junctions between the cells relax, and the barrier becomes more permeable than it should be. Bacterial fragments, particularly the outer wall components of certain gut bacteria, cross into the circulation in quantities they never should.

The immune system re-calibrates upward. Those fragments are potent immune triggers. They activate the innate immune system and raise the systemic inflammatory tone, a permanent low background hum of inflammatory signalling everywhere in the body.

Now put that hum into a hair follicle in your armpit. The follicular lining is already prone to overgrowing and plugging. Add a systemically primed immune system and the rupture that follows is not a small event, it is an explosion. Your skin is not sick. Your skin is downstream.

That is the gut-skin axis. It is not a metaphor. It is a supply chain.

The Crohn's overlap, which tells you something important

HS and Crohn's disease occur together far more often than chance allows. They share inflammatory pathways. They share tunnelling, fistulising behaviour. They share sites, including the perianal region. They respond to some of the same immune-directed drugs.

Two diseases that look completely different, one in the skin and one in the bowel, behaving like relatives. The reason is that they are not really two diseases of two organs; they are two expressions of the same immune and barrier dysfunction. Why antibiotics help, then make the ground worse

Antibiotics do help HS in the short term, partly by clearing secondary colonisers and partly because some have a direct anti-inflammatory effect. That relief is real, and it is why the cycle is so hard to see.

But repeated broad-spectrum courses strip the microbial diversity that keeps the gut barrier intact and the immune system calibrated. Dysbiosis deepens. Permeability rises. The systemic inflammatory tone climbs. The substrate the disease grows in gets richer, even as this month's lesion settles.

That is why each course seems to help a little less, and why the gap between courses gets shorter. You are treating the surface with something that degrades the source. This is not a reason to stop a medication you have been prescribed. It is a reason to fix the source so that you eventually need it less.

What driver correction does here

Treatment is built from your driver profile, gut health, hormonal balance, immune regulation, metabolic function, stress and cortisol, not from the diagnosis name. Where the gut is the dominant driver, the sequence matters enormously.

Phase L, Lowering the Load, oral, four to eight weeks: Inflammatory Load Reduction formulations bring systemic inflammatory signalling down so the skin stops reacting so violently.

Honest limits

Gut correction is slow, and it does not photograph well. The first change is usually internal, digestion, energy, bowel pattern, before the skin follows. Expect the Partial Improvement Plateau in weeks three to six, where the early lift stalls; this is not failure, and it is the point people abandon the protocol.

Not every HS patient is primarily gut-driven. That is what the driver assessment is for.
